In simple-simple terms:

If I want to jack up an AC voltage I can use a transformer.

If I want to jack up a DC voltage is my only choice a transistor/valve?

If I want to jack 9 volts to something huge, like 500,000 volts, is the option to cascade transistors?

You can generate AC from the DC source, with an oscillator or another form of switching device, then transform it way up with a television flyback transformer or car coil or something similar. Rectify it back with a high voltage rectifier, or possibly a series string of them to get breakdown voltage high enough.
