The midhorn flatpack kit includes the four flare sides and the throat plate plus Miller dowels for easy assembly. You don't need clamps with this setup because the Miller dowels hold everything in place.

Cost depends on the wood chosen. A kit like you're talking about would be about $350-400, depending on the wood you pick.
