As for acceptable height, I don't think there are any hard-and-fast rules, no formulas for setting it. This one is entirely subjective. The point is you don't want the perceived sound source to move in the vertical as it passes through center when panning left to right or vice versa. You don't want the dialog content to be at a different height than the left/right mains. You want a convincing illusion that the sound sources match the corresponding visual source locations.
