I want to build the Pi midhorn but space is an issue. What would be the disadvantages of mounting the midhorn vertically in order to have a narrow cabinet, with a 10" or 12" sealed subwoofer below it? Would I be able to integrate a tweeter properly?

Don't mount the midhorn vertically because then it wastes energy on vertical dispersion, which in turn, causes reflections off the floor and ceiling. If you want a narrow pattern, then go with a narrow flare. You might try a smaller horn using a smaller driver, but that may limit your lower cutoff and/or efficiency. If space is a problem, that may be your only choice other than a direct radiator. Honestly, if I have to reduce space, I prefer a direct radiator. When a horn is made too small, performance really suffers.
