back row:
6X5GT Rect.
6SK7 1st I.F.
6V6GT OUTPUT
middle row:
6SA7 Mixer
6SF7 2nd DET. & 2nd I.F.
front row:

From that tube set, I'd say it was probably made in the 1950's.
